Does the Apple Watch measure blood pressure? No. The Apple Watch does not measure blood pressure, and neither does any other consumer smartwatch you can buy today. None of them can take a cuff-grade blood pressure reading from your wrist alone.

What the Apple Watch does offer is something different: hypertension notifications. The watch looks at about 30 days of sensor data while you sleep and checks for patterns linked to chronic high blood pressure. That distinction matters. A cuff squeezes your arm and measures pressure directly. ## Why can't the Apple Watch measure blood pressure directly?

A home blood pressure cuff uses a method called oscillometric measurement. The cuff inflates until it squeezes the artery in your upper arm shut, then slowly deflates. A sensor inside the cuff watches the tiny pressure pulses in your arm as blood flow comes back. From those pulses, the device calculates your systolic pressure (the top number, when your heart pushes blood out) and your diastolic pressure (the bottom number, when your heart relaxes). It is a direct, physical measurement of the pressure inside your arteries.

The Apple Watch works completely differently. It uses optical heart sensing, also called PPG, short for photoplethysmography. Green LED lights shine into the skin on your wrist, and a sensor measures how much light bounces back. Every time your heart beats, blood volume in your wrist changes, and so does the amount of reflected light. That is how the watch counts your heart rate and tracks your heart rhythm.

Here is the problem: PPG measures blood volume, not pressure. Volume and pressure are related, but not in a fixed way. Your arteries stiffen with age, your hydration shifts, the watch sits at a different height than your heart, and the fit of the band changes. All of that distorts any attempt to turn volume into a pressure number. Getting a real blood pressure reading from the wrist would require inflating a cuff there, and no consumer watch has one. Some tech companies have researched blood pressure estimation from optical sensors, but nothing on the market today is accurate enough or cleared to replace a cuff.

So Apple took a different route. Instead of trying to measure blood pressure, the watch looks for patterns in its existing sensor data that tend to show up in people with chronic high blood pressure. That is the hypertension notifications feature.

## What the hypertension notifications actually do

The feature launched with watchOS 26 in September 2025 and was cleared by the U.S. FDA. According to Apple's official support page, here is how it works:

The watch analyzes data from its optical heart sensor over 30-day evaluation periods, starting from the day you set up the feature. Most of the relevant data is collected while you sleep, when your body is at rest and readings are cleanest. The algorithm is looking for a pattern consistent with chronic high blood pressure, meaning hypertension that persists over weeks, not a single stressful day.

If the feature detects that pattern, you get a notification on your watch and in the Health app on your iPhone. The alert says the watch identified patterns related to hypertension within the last 30 days of your heart data.

When you get an alert, Apple prompts you to do two things:

1. Set up a Blood Pressure Log in the Health app.
2. Use a third-party blood pressure cuff to measure and track your blood pressure for 7 days.

That last step is important. Apple is telling you outright: the watch flags a possibility, the cuff confirms or rules it out. You should also discuss the notification and your cuff readings with your healthcare professional.

To even turn the feature on, you have to meet the eligibility rules from Apple's support page:

- An Apple Watch Series 9 or later, or Apple Watch Ultra 2 or later, with the latest version of watchOS. It is not available on Apple Watch SE.
- An iPhone 11 or later with the latest version of iOS.
- Wrist Detection turned on.
- You must be 22 or older.
- You must not be pregnant. If you have an active pregnancy logged in the Health app, you cannot enable the feature.
- You must not have been diagnosed with hypertension already. The feature is designed to screen people who do not know they have high blood pressure. If you already have a diagnosis, your doctor is already managing your blood pressure, and the feature is not intended to help with that.

## How accurate is the Apple Watch at detecting high blood pressure?

Honest answer: it is a useful screening tool, but it misses a lot of cases, and it can raise alarms that turn out to be nothing.

Apple published a validation study, which was part of its FDA clearance. The headline numbers:

- Sensitivity of 41.2 percent. That means in the validation study, only about 4 in 10 people who actually had undiagnosed hypertension received an alert. The other 6 in 10 got no alert at all.
- Specificity of 92.3 percent. Among people without hypertension, about 92 percent correctly got no alert. That means most alerts are not false alarms, but some are.
- Sensitivity was higher for stage 2 hypertension, the more severe level, because stronger high blood pressure leaves a clearer footprint in the sensor data.

Independent researchers have looked at these numbers too. A 2026 study in JAMA by Cohen and colleagues applied them to a nationally representative sample of US adults. In that model, about 69 percent of alerts pointed to true hypertension once confirmed with cuff measurements. The researchers also flagged the flip side: false reassurance. Someone with high blood pressure who never gets an alert might wrongly assume they are fine.

Apple's own support page is candid about the limits. It states that the feature is not intended to diagnose, treat, or help manage hypertension, and that not all people with hypertension receive a notification.

So the practical takeaway: no alert does not mean your blood pressure is fine, and an alert does not mean you have hypertension. It means check with a cuff.

## What the Series 12 changes

On September 9, 2026, Apple announced the Apple Watch Series 12, built around an all-new Health Sensing System. It ships September 18 and starts at $399. Here is what is new, and what is not.

The Series 12 has new optical and electrical heart sensors plus the S11 chip. Apple says it delivers the most accurate heart rate sensing in a wearable. That claim comes from an Apple-conducted study of more than 1,000 participants that compared the watch against leading competitors. The watch now measures your heart rate every five seconds, all day, instead of sampling less often. And HRV, or heart rate variability, is now measured up to 24 times more often. It comes in two variants. Recovery HRV tracks daily stress and recovery signals. Overall HRV tracks broader cardiovascular health.

Those upgrades matter for features downstream of heart sensing, like the new readiness score, overnight and daytime vitals views, and workout tracking. More frequent, cleaner readings generally mean better trend detection.

But here is the part that matters for your question: blood pressure on the Series 12 is still the same story. Apple made no announcement of a blood pressure sensor or cuff-free blood pressure measurement. The hypertension notifications feature still works the same way, from 30 days of passive data, and it still requires you to confirm any alert with a cuff. A better optical sensor may improve the data the algorithm has to work with over time, but it does not change the physics. Volume signals are still not pressure measurements.

So if you were hoping Series 12 finally reads blood pressure, it does not. Nobody's watch does.

## Best ways to actually track blood pressure

If you want real blood pressure numbers, you need a cuff. Here is how to do it right:

- Buy an upper-arm cuff monitor, ideally one validated for home use. Upper-arm cuffs are more accurate than wrist cuffs.
- Measure at the same times each day, such as morning and evening. Sit still for five minutes first, feet flat, arm supported at heart level.
- Take two readings a minute apart and log both.
