Here is the short version: **the Apple Watch ECG app records a single-lead heart rhythm in 30 seconds and checks it for signs of atrial fibrillation, the most common serious heart rhythm disorder.** A result is either sinus rhythm, AFib, or something inconclusive, and every result comes with real limits you should understand before you take your first reading.

The feature is genuinely impressive and genuinely misunderstood. In Apple's clinical validation, the app agreed with cardiologist-read 12-lead ECGs on classifiable recordings about 98% of the time for rhythm classification. But a single-lead wrist recording is not a full cardiac workup, an inconclusive result is common in the real world, and no version of this app can tell you that nothing is wrong. Here is how to use it properly.

## The short answer

- **What it does:** Records an ECG similar to a Lead I single-lead tracing. Touching the Digital Crown completes a circuit with the sensor on the watch back, so the watch can measure the heart's electrical signals.
- **Which watches have it:** Apple Watch Series 4 or later and all Ultra models. The SE line lacks the electrical heart sensor, and Apple Watch For Your Kids does not support the app.
- **Who it is for:** People 22 and older. It checks for AFib between 50 and 150 beats per minute depending on app version, and it is not recommended if you have other known arrhythmias or a pacemaker or ICD.
- **How to take one:** Open the ECG app, rest your arms, hold your finger lightly on the Digital Crown for 30 seconds, and read the classification.
- **What results mean:** Sinus rhythm means uniform beating at 50 to 100 BPM for that recording only. AFib means irregular beating and deserves a call to your doctor. Inconclusive means the recording could not be classified, which happens often and has many causes.
- **What it cannot do:** Detect heart attack, stroke, blood clots, high blood pressure, or other arrhythmias. Symptoms always outrank the screen.

## Which watches can take an ECG

The electrical heart sensor arrived with Series 4 in 2018, and every flagship generation since includes it:

- Apple Watch Series 4, 5, 6, 7, 8, 9, 10, and 11
- Apple Watch Ultra, Ultra 2, and Ultra 3

Not included: the SE line in all generations. The SE omits the electrical heart sensor to hit its price, so it offers high and low heart rate notifications but no ECG and no on-demand AFib check. If AFib detection is why you are buying, the SE is the wrong model.

## Setting up and taking a reading

Setup happens once, in the Health app on your iPhone: tap Search, then Heart, then Electrocardiograms (ECG), then Set Up ECG App. After that the ECG app lives on your watch.

When you want a reading:

1. Sit down and rest your arms on a table or in your lap.
2. Make sure the watch is snug on the wrist you selected during setup, with the back crystal touching skin.
3. Open the ECG app on the watch.
4. With the hand opposite the watch, hold your finger on the Digital Crown. Rest it there; do not press.
5. Stay still for 30 seconds while it records.
6. Read the classification, optionally add symptoms, then save.

The reading lands in the Health app under Heart, Electrocardiograms (ECG), where you can see the waveform and export a PDF for your doctor. That PDF matters more than people expect: cardiologists can compare serial recordings over months, which turns a gadget into actual clinical context.

Take readings when you feel symptoms like a rapid or skipped heartbeat, when you just want a periodic baseline, or after an irregular rhythm notification asks you to record one.

## How to read each result

### Sinus rhythm

The normal result. Your upper and lower chambers beat in a uniform pattern between 50 and 100 BPM. Two caveats from Apple are worth memorizing. First, the result applies to that recording alone; hearts alternate between clean and messy tracings, especially with paroxysmal arrhythmias that come and go. Second, sinus rhythm does not certify health. It means this 30-second sample looked organized.

### Atrial fibrillation

AFib means the recording caught an irregular pattern, the signature of the most common serious arrhythmia. Untreated AFib raises stroke risk substantially because poorly coordinated atria let blood pool and clot. An AFib result without a prior diagnosis warrants contacting your doctor soon, not panicking tonight. Bring the exported PDF.

One technical detail changes what the app can see: ECG app version 1 classifies rhythms between 50 and 120 BPM, while version 2 extends that range to 50 to 150. If you exercise hard, the version difference explains some confusing results.

### Low or high heart rate

Below 50 BPM, or above the version's ceiling, the app declines to classify. Neither number automatically means danger. Endurance athletes commonly idle under 50, and medications influence rate in both directions. High rates trace back to exercise, stress, alcohol, dehydration, infection, caffeine, or an arrhythmia including AFib itself. Unexpected extremes deserve a medical conversation regardless of what the app says.

### Inconclusive

The recording happened, but no classification was possible. Common causes include heart rate between 100 and 120 BPM without AFib on version 1, a pacemaker or implanted defibrillator, rhythms the app is not designed to recognize, poor contact or movement, and certain physiological conditions, such as heart positioning, that limit signal in a small percentage of people. One inconclusive usually means retake it under better conditions. Repeated inconclusives deserve a doctor visit, since some of the causes are themselves findings.

### Poor recording

Version 2 splits the old inconclusive bucket further. Poor Recording flags artifact: loose band, sweaty skin, movement, nearby plugged-in electronics. Clean and dry the wrist, tighten the band, sit still, and try again.

## What the evidence says

Two bodies of research frame expectations.

In Apple's clinical validation trial of roughly 600 subjects, classifiable ECG app recordings matched cardiologist-interpreted 12-lead ECGs with 98.3% sensitivity for AFib and 99.6% specificity for sinus rhythm. Those numbers describe ideal conditions: seated, still, cooperative signal. Apple itself notes real-world use produces more inconclusive strips than the trial did.

At population scale, the Stanford-led Apple Heart Study enrolled 419,297 participants and published in the New England Journal of Medicine in 2019. Only 0.52% ever received an irregular pulse notification from passive monitoring. Among notified participants who wore a medical-grade ECG patch afterward, 34% showed AFib, and simultaneous patch-and-watch analysis confirmed AFib during 84% of subsequent notifications. The design favors few false alarms over catching everything, which is the right trade for a consumer device but means silence never proves absence.

## The honest limitations

Keep these from Apple's own documentation in mind:

- The ECG app cannot detect a heart attack. Chest pain, pressure, or tightness means calling emergency services immediately, no matter what the last reading said.
- It cannot detect stroke risk directly, blood clots, high blood pressure, congestive heart failure, high cholesterol, or arrhythmias other than AFib.
- It takes snapshots, not continuous monitoring. Paroxysmal AFib hides between recordings, and a normal strip during a quiet week proves little.
- Do not use it with a pacemaker or ICD, and do not record during workouts or near strong electromagnetic fields.
- Interpretations are potential findings, not diagnoses. Decisions belong to you and a clinician together.

### Is the Apple Watch ECG accurate?

For classifiable recordings in controlled testing, yes: 98.3% sensitivity for AFib and 99.6% specificity for sinus rhythm against 12-lead interpretation. Real-world accuracy is lower because of motion, sweat, and unclassifiable signals, and a wearable single-lead snapshot cannot substitute for clinical evaluation.

### Can the Apple Watch ECG detect a heart attack?

No. Apple states this plainly in the product documentation. Heart attacks involve muscle damage the single-lead app cannot assess. Cardiac symptoms mean emergency services, immediately.

### Why does my Apple Watch say inconclusive?

Usually fixable physics: loose fit, damp skin, movement, or a heart rate outside the classification range. Sometimes it is a pacemaker, another arrhythmia, or body-related signal limits. Retake once under better conditions; recurring inconclusives belong in a doctor conversation.
