Your Apple Watch just told you it detected signs of possible sleep apnea. Take a breath. This notification is not a diagnosis, and it does not mean something is wrong with you right now. A possible sleep apnea notification means your watch measured elevated breathing disturbances on enough nights over a 30-day period. That pattern suggests you should talk to a doctor about sleep apnea. That is the whole message. The next step is a conversation with your doctor, and maybe a sleep study. Both are routine, and sleep apnea is very treatable.

You are probably wondering how the watch even knows this, whether you can trust it, and what you say when you call your doctor's office. This article walks through all of it, using Apple's own documentation and the validation study Apple published when the feature launched.

The short answer

  • The watch uses its accelerometer to track breathing disturbances while you sleep. It does not measure your airway or your oxygen directly.
  • You got the notification because at least half of your recorded nights in a 30-day window showed elevated breathing disturbances, based on at least 10 nights of data.
  • It is a screening signal, not a diagnosis. Apple's feature is explicitly not intended to diagnose, treat, or manage sleep apnea.

What the Apple Watch actually measures

Here is the part most people get wrong: the Apple Watch never looks at your breathing directly. It measures motion.

Inside every Apple Watch is a three-axis accelerometer, a tiny sensor that detects movement in three directions. When you wear the watch to bed, that sensor picks up two kinds of motion: the big movements of your body shifting position, and very small movements linked to your breathing. Apple built an algorithm that reads this motion data and estimates how often your breathing pattern gets interrupted during sleep. These interruptions are called breathing disturbances.

The Health app sorts each night into one of two buckets: Elevated or Not Elevated. A few disturbances a night is normal. Alcohol, a cold, and some medications can all push the number up temporarily. That alone will not get you a notification.

One important detail: breathing disturbances are not the same thing as the apnea-hypopnea index (AHI) that sleep doctors use. AHI counts actual apnea and hypopnea events per hour, measured with clinical equipment. The watch produces an estimate in similar units, but Apple's own study is clear that the two are not equivalent.

Why you got the notification

Apple's notification logic is deliberately conservative. Here is how it works, per Apple's documentation:

  1. The feature needs at least 10 nights of sleep data with breathing disturbance measurements within a 30-day window. Fewer than 10 nights, and it stays silent, because there is not enough information.
  2. If at least 50% of those nights are classified as Elevated, you get a notification of possible sleep apnea.
  3. The check runs every 30 days. If you did not get a notification this month, the algorithm keeps watching and checks again next month.

So the notification means: across a full month of monitoring, elevated breathing disturbances were consistent, not a one-off bad week. That is the signal worth acting on.

Apple designed the threshold this way on purpose. The feature checks again and again over time, so a false alarm would repeat too. Apple chose settings that favor ruling people out over catching every case. More on what that means for accuracy below.

How accurate is it?

Apple published the numbers, and they are worth understanding because they answer two different questions.

First: how often is the notification right? In Apple's clinical validation study of 1,448 participants, the notification had a sensitivity of 66.3% and a specificity of 98.5%. Those words need unpacking:

  • Sensitivity (66.3%): of people who truly had moderate to severe sleep apnea, about two-thirds got a notification. Roughly one-third did not. The feature misses cases, especially moderate ones (sensitivity for the moderate category was 43.4%). It was much better at catching severe cases: 89.1% of people with severe sleep apnea got flagged.
  • Specificity (98.5%): of people who did not have moderate to severe sleep apnea, 98.5% correctly got no notification. In the normal category, specificity was 100%: every single person who got a notification had at least mild sleep apnea.

Read together, this tells you something reassuring: a notification is rarely a fluke. If your watch flagged you, the odds are good that you have at least some degree of sleep apnea, though only a sleep study can confirm it and say how severe it is.

Second: how close are the nightly numbers to real measurements? In the study, 91.4% of participants had breathing disturbance values that fell within an acceptable error zone compared with a home sleep test. So the nightly graph is a reasonable estimate, not a clinical measurement.

The limits to keep in mind:

  • The feature is cleared for people 18 and older who have not already been diagnosed with sleep apnea.
  • Not everyone with sleep apnea gets a notification. Apple says this plainly: no notification does not mean no sleep apnea.
  • The feature does not take your symptoms into account. If you have symptoms, you should see a doctor regardless of what the watch says.
  • It cannot diagnose, treat, or help manage sleep apnea. Only a clinician with proper testing can do that.

For context on how wrist sensors compare with clinical testing more broadly, sleep medicine researchers are still refining the technology. One study of 122 people found a wrist wearable correctly measured breathing disturbances in 85% to 89% of participants, but standard diagnosis still requires a sleep study that measures oxygen at the fingertip, airflow, and other signals a watch cannot capture.

What to do now: a concrete plan

Step 1: Export your report

Open the Health app on your iPhone, tap Browse, then Respiratory, then Breathing Disturbances. Scroll down and tap Export PDF. Apple built this report for exactly this purpose. It gives your doctor the month-by-month picture in a format they can read quickly.

Step 2: Book a regular doctor's appointment

Call your primary care doctor. You do not need an emergency visit or an urgent care trip. Say something like: "My Apple Watch flagged possible sleep apnea after monitoring my breathing for a month. I brought the report. Can we discuss whether I need a sleep study?" That sentence gives them everything they need to start.

Step 3: Know what a sleep study is

A sleep study is an overnight test that measures what is actually happening in your body while you sleep. Your doctor will recommend one of two versions:

  • Home sleep study. You take a small kit home and sleep in your own bed with a few sensors: a finger clip that measures blood oxygen, a thin tube in your nose that measures airflow, and a chest sensor for heart rate. It is simpler, cheaper, and works well for the most common type, obstructive sleep apnea.
  • In-lab sleep study (polysomnography). You sleep overnight at a sleep center with more sensors, including stickers on your scalp that measure brain waves. This is the full picture, and doctors prefer it when they suspect something other than straightforward obstructive sleep apnea.

Either way, the results give your doctor an AHI number, which is how many breathing events you have per hour. Five or more events per hour with symptoms, or 15 or more without symptoms, meets the threshold for a sleep apnea diagnosis.

Step 4: Act faster if you have certain symptoms

Most notifications can wait for a routine appointment. See a doctor sooner, days not months, if any of these apply:

  • Someone has watched you stop breathing at night, or heard you gasp, choke, or snort awake
  • You wake up gasping for air
  • You get a full night of sleep but still feel exhausted, can't concentrate, or fight off sleepiness during the day

These are classic sleep apnea symptoms, and a watch notification plus symptoms is a stronger case for prompt testing. Untreated sleep apnea raises the long-term risk of high blood pressure, heart attack, and stroke. That is exactly why it is worth checking. Treating it, often with a CPAP machine or a dental appliance, makes a real difference.

What to expect at the appointment

Your doctor will ask about snoring, daytime sleepiness, your weight history, and whether anyone has observed pauses in your breathing. They may examine your throat and neck. Then they will likely order a sleep study. That is the whole path. If the study confirms sleep apnea, treatment options include CPAP, oral appliances, position therapy, and in some cases surgery. Most people who stick with treatment report sleeping and feeling dramatically better.

Can the Apple Watch diagnose sleep apnea?

No. The feature is a notification tool cleared by the FDA to flag signs of possible moderate to severe sleep apnea in people who have not been diagnosed. Apple's documentation states it is not intended to diagnose, treat, or aid in the management of sleep apnea. Diagnosis requires a sleep study interpreted by a clinician.

Why did I get a possible sleep apnea notification?

Your watch recorded at least 10 nights of breathing data in a 30-day period, and at least half of those nights showed elevated breathing disturbances. That pattern is associated with moderate to severe sleep apnea, which is why Apple surfaces the alert. Does the notification mean my sleep apnea is severe?

Not necessarily. The notification targets moderate to severe sleep apnea, but only a sleep study can tell you where you actually fall. Apple's own data shows a gap here. The feature catches severe cases much more reliably than moderate ones: 89.1% versus 43.4%. So a notification is a meaningful signal. But severity is a clinical measurement, not a watch estimate.

I already know I have sleep apnea. Does this feature help me?

No. The feature is intended only for people who have not been diagnosed. If you have already been diagnosed, your breathing disturbance data may still be interesting to glance at, but the notification algorithm is not designed or cleared for managing a known condition. Follow up with your sleep specialist about your treatment instead.

What if I got no notification but my partner says I stop breathing at night?

Trust the human. Witnessed breathing pauses are one of the strongest signs of sleep apnea, and Apple's feature misses roughly a third of moderate to severe cases. Tell your doctor what your partner observed. A symptom report plus a sleep study is the standard path, with or without a watch notification.
